They probably are fairly busy and don't realize they hadn't put them in yet before handing it over. If I'm ordering something from a drive through that I know I'll need napkins for I always ask if there are napkins and for more just to be safe. Kitchens and restaurants are busy and loud places and it can be easy to get ahead of yourself as speed is pushed a lot in the industry and it's easy to miss something. Never hurts to just check.
