Found live crawling insects on the floor and improperly insulated wirings by the table, which can easily be touched by children. Food was not good either.

I don't recall beer. 99.9% sure no beer.
Yes. They have several vegetarian options.
Around $10 but the standard size of bowl is really large. No need to order extra large size.
